By submitting this form, you confirm that all plan documents related to your client’s benefits package, along with important contact information, are 100% complete and will be used to generate your Enrollment Meeting on Demand™ script and final video production.

Submission & Proofing Process:

• Document-Based Generation: The script is generated using the benefit summaries and SBCs you provide. These documents determine the accuracy and content of the final product.
• Proofing & Edits: Once processing is complete, you will receive your proofing form. You will have 72 hours to review and submit any edits. This timeframe allows us to maintain exceptional turnaround times while serving all clients effectively.
• Failure to Proof: If we do not receive requested edits within this window, you acknowledge that there are no changes needed and we will proceed with final production. (Don’t worry—93% of brokers say there’s no need to make edits at all!)
• Late Changes: If proofing is not completed within 72 hours and your group has already been placed into production, a $295 reprocessing fee will apply to restart the process.
